Understanding Italian is not the same as speaking it
A lot of Italian learners are heritage learners. You grew up hearing it at Sunday lunch, you understand the jokes, and then you reply in English. The words are in there; what is missing is the habit of producing them.
Italian also hides its difficulty in small, fast choices: essere or avere in the passato prossimo, where the pronoun goes, when "ci" and "ne" appear. Learners who study these rules still trip on them in conversation until they have said them out loud many times.
